Responsibilities

• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track treatment goals.
• Use standardized assessments and evidence-based practices to support interventions.
• Assess progress and adjust treatments, recording updates per regulatory requirements.
• Evaluate treatment outcomes and adjust services; refer as needed.
• Provide consultation to patients, families, caregivers, and other providers.
• Communicate with supervisors and health team members on progress, barriers, and plans.
• Provide comprehensive discharge summaries.
• Document regulatory information in electronic records and record treatment minutes.
• Participate in trainings, in-services and conferences.
• Supervise and support physical therapist assistants, aides and students.
• Adhere to company policies, procedures and code of conduct.
