Job summary

Sweetwater Care in Colorado is seeking a licensed Physical Therapist to join our rehabilitation team. You will assess movement dysfunction, develop individualized treatment plans, and guide patients through therapeutic massage to ease pain and support healing.

You will supervise PT assistants, maintain detailed records, and collaborate with physicians, surgeons, and other health professionals. Active state licensure is required. Sweetwater Care is an equal opportunity employer.

Qualifications

• Active State Licensure required.
• Experience supervising PT assistants and rehab aides desired.
• Supervision of students may be required.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Diagnose movement dysfunction by observing patients.
• Develop individualized treatment plans.
• Coach patient and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments to support interventions.
• Assess progress and adjust treatments as needed.
• Evaluate treatment outcomes and modify plans accordingly.
• Provide counseling to patients and families.
• Communicate progress with supervisor and health team members.
• Prepare comprehensive discharge summaries.
• Document regulatory information in electronic system.
• Participate in trainings and in-services.
• Supervise PT assistants and rehab staff.
• Adhere to company policies and code of conduct.
