Job summary

Sweetwater Care in Montana is seeking a Physical Therapist to lead patient-focused rehabilitation programs. You will design and implement individualized treatment plans, guide patients through therapeutic exercises, and work with a multidisciplinary team to optimize recovery.

Qualifications

• Active state licensure as a Physical Therapist.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Review medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess patient progress and adjust treatments per regulatory and clinical requirements.
• Evaluate outcomes and modify services based on patient evaluation and referrals.
• Provide consultation to patients, families, caregivers and other providers related to physical disorders.
• Communicate with supervisor and health team regarding progress, barriers and plans.
• Provide discharge summaries per regulatory requirements.
• Document all regulatory information in electronic records and maintain patient files.
• Participate in trainings, in-services and conferences.
• Supervise and support PTAs, rehabilitation aides and students.
• Adhere to company policies and code of conduct.
