Job summary

JAG Physical Therapy is seeking a dedicated Physical Therapist to assess patients, design and implement treatment plans, and collaborate with a multi-disciplinary team across multiple facilities. You will contribute to a high-energy, patient-centered environment focused on healing, functional improvement, and patient success.

Ideal candidates hold a PT license in the state, have strong communication, and are comfortable with EMR systems.

Qualifications

• Licensed PT in the state practicing in
• Completed CEU requirement as applicable in NY/NJ
• Excellent oral and written communication
• Strong clinical decision-making and work ethic

Responsibilities

• Collect and record patient care data per format and guidelines
• Prepare evaluations and plan of care with goals and prognosis
• Develop and adjust treatment plans with the interdisciplinary team
• Provide education and training to patient and support system
• Oversee PTA, ATC, PT aides and contribute to documentation
