Job summary

Advocate Health Care in Libertyville, IL seeks a Physical Therapist to perform musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations and to establish and implement individualized treatment plans aimed at achieving functional goals. You will monitor outcomes, adjust plans as needed, and discharge patients with written home programs and family education.

Strong IL/WI licensure and a Doctorate in Physical Therapy are required, along with excellent communication and teamwork

Qualifications

• Doctorate degree in Physical Therapy required.
• Current IL or WI Physical Therapy license required.
• Excellent communication and organizational skills required.
• Ability to develop and implement patient care plans and work in a team.

Responsibilities

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establish and implement individualized treatment plans based on evaluations and goals.
• Monitor outcomes and adjust plans as needed; provide status updates to physicians and teammates.
• Complete discharge planning with home programs and family education; coordinate referrals for community services.
• Document all patient care and progress per policy; ensure productivity standards are met and proper billing.
• Maintain safety and infection control practices; supervise staff as requested.
