Qualifications

• Graduate of an accredited Physical Therapy program.
• Oregon Physical Therapist License.
• AHA BLS within 30 days.

Responsibilities

• Interviews patient, family, and caregivers to obtain information on function and history.
• Performs tests, measurements and evaluations as indicated.
• Establishes measurable/functional treatment goals.
• Plans and prepares written treatment programs and administers treatments as prescribed.
• Evaluates treatment effectiveness and adjusts plan for maximum benefit.
• Monitors patients, supervises staff, and records documentation; reports changes to physician.
