Job summary

Qualifications

• Doctorate in Physical Therapy (DPT) or equivalent.
• Graduate of an accredited physical therapy program.
• Current Ohio Physical Therapy license; BLS certification.

Responsibilities

• Performs PT evaluation and establishes treatment plan for patients referred by a provider.
• Documents evaluation results, interventions, patient progress, and discharge records.
• Maintains proper billing for PT services.
• Delegates tasks to PTA and co-signs documentation as needed.
• Communicates with health care providers regarding treatment, continuity of care, and discharge planning.
