Job summary

Mahoning Post Acute Nursing & Rehabilitation is seeking a compassionate Physical Therapist to join our Skilled Nursing Facility team for a full-time position. The role focuses on helping older adults regain function, mobility, and quality of life within a collaborative, patient-centered environment.

The therapist will perform comprehensive evaluations, develop individualized plans, provide therapies, and document progress in accordance with regulatory guidelines.

Qualifications

• Active state Physical Therapist license.
• Experience in geriatrics or skilled nursing preferred; new grads welcome.
• Excellent communication and documentation skills.
• Ability to work in a collaborative, interdisciplinary team.

Responsibilities

• Perform comprehensive evaluations for residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions.
• Develop individualized treatment plans tailored to resident goals, functional needs, and SNF regulatory guidelines.
• Provide therapeutic interventions including strengthening, balance training, gait training, transfer training, pain management, and functional mobility retraining.
• Collaborate closely with nursing, occupational therapy, speech therapy, and social services to support interdisciplinary care.
• Document evaluations, daily notes, progress reports, and discharge summaries in accordance with Medicare, state, and facility requirements.
• Educate residents and caregivers on safe mobility, fall prevention, and home exercise programs.
• Maintain a safe and supportive therapy environment.
