Qualifications

• Active license to practice in Minnesota.
• 2 years of acute/subacute/homecare experience preferred.

Responsibilities

• Conducts evaluation and documents findings in the medical record.
• Develops and implements age-appropriate functional goals and treatment plans.
• Selects interventions based on indications/contraindications and patient function.
• Delivers treatment aligned with current PT practice.
• Supervises and delegates care to support personnel.
• Documents progress and treatment outcomes accurately and timely.
• Communicates with the interdisciplinary team about progress and plan changes.
• Address coverage requirements for casual/call shifts.
