Job summary

Advocate Health in North Carolina is seeking a licensed Physical Therapist to evaluate patients, design functional treatment plans, and ensure quality outcomes. You will work within hospital standards, document progress, and communicate clearly with patients, families, and the care team.

The requirements include a B.S., M.S., or D.P.T from an accredited PT program, current NC licensure, and CPR (BLS Level C).

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of applicable state
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.
• Provides patient care assessment and treatment, ensuring evaluations and treatments are comprehensive and aligned with standards of practice.
• Maintains accurate documentation, completes evaluations, progress notes, discharge notes, and ICD-10 coding as appropriate.
• Ensures documentation is legible.
• Meets department financial and productivity standards and adjusts schedules to meet patient and department needs.
• Participates in process management to optimize quality and efficiency of services and communicates progress barriers to supervisor.
• Promotes professional development and education of health care workers and provides representation on committees.
• Communicates effectively with patients, physicians, staff, and external customers; promotes open communication and informs supervisor of issues and resolutions.
• Provides appropriate patient care with age/developmental considerations and maintains competence accordingly.
