Advocate Health is seeking a licensed Physical Therapist to interpret referrals, perform comprehensive musculoskeletal and related evaluations, and develop individualized treatment plans. You will monitor progress, adjust plans, and communicate with physicians to ensure optimal patient outcomes.

The role includes discharge planning, patient/family education, documentation, productivity, and adherence to billing policies.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state of practice plus BLS certification.

Responsabilités

• Interpret referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establish a plan of care and implement treatment based on evaluation results and functional goals.
• Monitor and adjust treatment plans based on patient response; provide status updates to physicians and teammates.
• Complete discharge planning with home programs, patient/family teaching, and referrals as needed.
• Document care and progress; manage statistics, billing, and scheduling; follow insurance guidelines.
• Maintain safety, infection control, and proper use of modalities and equipment.
• Supervise PT assistants or rehab staff as needed and participate in performance improvement activities.
