Job summary

BAYADA Home Health Care, Inc. is seeking a Physical Therapist for per diem, part-time, or full-time home health visits in Bushkill and surrounding areas. A PA PT license and at least one year of experience are required.

You will perform assessments, develop care plans, and educate clients to promote health, safety, and independence in the home setting, collaborating with a multidisciplinary team.

Qualifications

• Current Pennsylvania Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program (or equivalent).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, transfers, balance, and strength.
• Implement wound care and topical interventions as indicated.
• Develop and implement individualized care plans with PT interventions.
• Educate clients and families on rehabilitation activities for health and independence.
• Accurately document observations and interventions using EMR tools.
