Job summary

You will collaborate with physicians, social workers and community organizations, educate families, and monitor progress to improve health outcomes and reduce readmissions.

Qualifications

• Must hold a current Ohio nursing license (RN or ASN)
• Experience coordinating care across healthcare providers and community resources
• Strong knowledge of Medicare/Medicaid programs and managed care
• Ability to assess medical, behavioral health, and social needs in home settings
• Proven skills in developing and updating individualized care plans
• Excellent documentation and communication with multidisciplinary teams

Responsibilities

• Conduct comprehensive member assessments to identify medical, behavioral health, functional, and social needs.
• Develop, implement, and regularly update individualized care plans.
• Meet with members in homes, clinics, and community settings.
• Act as liaison between members, providers, and community organizations.
• Lead interdisciplinary care teams to coordinate member care.
• Coordinate transitions of care and follow up after hospitalizations to prevent readmissions.
• Connect members with housing, transportation, food, and social services.
• Educate members and families on Medicare/Medicaid benefits and preventive care.
