Job summary

Continuum of Care in Hamden, CT seeks a Direct Support Professional to assist individuals with intellectual disabilities in home settings. You will help with daily living activities, personal care, and community participation, delivering care with dignity and respect in line with each IP.

The role requires evening, weekend, and overnight availability. A CT driver’s license is needed, with CPR/First Aid and additional DDS/CoC certifications preferred.

Qualifications

• Bachelor’s or Associate preferred; HS diploma with related experience considered.
• Valid Connecticut driver’s license with clean record.
• CPR & First Aid, PMT and other required certifications through DDS/CoC.
• Computer skills to navigate agency electronic records and documentation.

Responsibilities

• Establish rapport and professional boundaries with individuals.
• Provide face-to-face hands-on support for ADL and personal care.
• Deliver services per IP and positive behavioral programming.
• Encourage involvement in social and community activities.
• Input data in Therap or designated electronic systems; track daily data.
• Attend staff meetings and complete assigned duties.
• Support individuals to access medical and community resources.
