Qualifications

• English proficiency required; bilingual skills beneficial.

Responsibilities

• Provide hands-on, person-centered support per ISP/IPP.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Document care notes accurately and timely; HIPAA compliant.
• Support transportation to appointments and community activities.
• Follow behavior support plans and participate in trainings.
