Job summary

Human Touch Home Health is seeking a compassionate Home Health Physical Therapist to join our team serving communities across the DMV. You will provide one-on-one, in-home therapy, design personalized plans, and help patients regain mobility and independence with flexible scheduling.

Qualifications

Active PT license in DC, MD or VA is required or multi-state licensure.
Valid driver's license with reliable transportation.
Strong clinical assessment, communication and documentation skills.
OASIS experience and WellSky familiarity are a plus.

Responsibilities

Perform PT evaluations, SOC, reassessments and discharge visits.
Create individualized care plans based on patient goals.
Deliver skilled PT services in patients' homes.
Educate patients and caregivers on exercises, mobility, balance and safety.
Monitor progress and adapt treatment plans accordingly.
Collaborate with physicians, nurses and the care team.
Document accurately using WellSky.
