Job summary

Human Touch Home Health seeks a compassionate Physical Therapist to join our growing team serving communities across Prince George's and Montgomery counties in Maryland. This PRN role offers flexibility and autonomy to deliver one-on-one care in patients' homes.

You'll build individualized treatment plans, regain mobility, and help patients return to daily activities, work, and independent living, while collaborating with a multidisciplinary team and leadership that supports your growth.

Qualifications

• Active Maryland Physical Therapist License Required.
• Valid Driver's License and Reliable Transportation.
• Home health experience preferred but not required.

Responsibilities

• Perform home health PT evaluations and SOC/discharge visits.
• Develop individualized plans of care based on goals and clinical needs.
• Provide skilled PT services in patients' homes.
• Educate patients and caregivers on exercises, mobility, balance, fall prevention, home safety.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, social workers and care team.
• Complete clinical documentation accurately using WellSky.
