Job summary

Human Touch Home Health is seeking a compassionate Physical Therapist (PT) to serve patients across Hyattsville, Mount Rainier, Brentwood, New Carrollton, and surrounding Prince George's County communities in ZIPs 20772, 20782, 20783, and 20785. This PRN role offers flexible scheduling and autonomy to develop personalized treatment plans in patients' homes.

Qualifications

Active Maryland Physical Therapist license required.
Valid driver’s license and reliable transportation.
Home Health experience preferred but not required.
OASIS experience preferred.
Experience with WellSky Home Health is a plus.
Passion for delivering exceptional patient-centered care.

Responsibilities

Complete physical therapy evaluations, SOC, reassessments, and discharge visits.
Develop individualized plans of care based on patient goals and clinical needs.
Provide skilled physical therapy services in patients' homes.
Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety.
Monitor patient progress and modify treatment plans as appropriate.
Collaborate with physicians, nurses, social workers, and the interdisciplinary care team.
Complete timely and accurate clinical documentation using WellSky.
