Job summary

NorthWest Senior and Disability Services in Oregon is seeking a qualified Case Manager to support Diversion and Transition services, helping seniors and people with disabilities avoid unnecessary nursing facility placement by guiding them toward home- and community-based options.

You will assess needs, develop and monitor transition plans, coordinate with ADRC resources, promote agency services, and build partnerships in the community while upholding privacy, safety, and regulatory requirements.

Qualifications

• Bachelor’s degree in social sciences or related field.
• Six years of combined education/experience in social services.
• Experience with seniors or disabilities is a plus.

Responsibilities

• Determine need for Diversion and Transition Services.
• Develop and implement transition plans.
• Monitor and adjust transition plans.
• Transition cases to ongoing Case Manager.
• Support and promote Diversion and Transition services.
• Promote ADRC concepts.
• Facilitate confidence in the Case Manager and programs.
• Protect consumers and reduce agency risk.
• Provide excellent customer service.
