Job summary

Options for Southern Oregon is seeking a Case Manager-Skills Trainer for Jail Diversion in Grants Pass, OR. The role supports recovery-focused services for individuals with serious mental illness, using jail in-reach strategies and evidence-based practices.

The position requires a bachelor's in a human services field or equivalent experience, MHACBO QMHA registry or eligibility, and the ability to drive and use an EHR system.

Qualifications

• Bachelor’s degree in human services or 3 years of relevant experience with mental health/disabilities.
• Must be QMHA on MHACBO registry or eligible to register.
• Proficiency with email, document creation, and MS Office; familiarity with EHR systems.

Responsibilities

• Assist clients in accessing community resources and develop transition plans.
• Coordinate jail diversion, PSRB and program-specific services.
• Teach life skills such as budgeting, emotional self-regulation and communication.
• Liaise with community partners to facilitate access to behavioral health services.
• Manage caseload and drive clients between sites as needed.
