Job summary

Pillars Inc is hiring a Full-Time Diversion and Prevention Case Manager to join the Client Services team in Appleton, WI. The role focuses on identifying, assessing, and delivering case management services to clients staying in temporary housing, with the aim of moving them into affordable, stable housing and avoiding shelter entry.

You will work a 40-hour week, using a trauma-informed approach, build relationships, and coordinate with community resources.

Qualifications

• Bachelors in Human Services related field or equivalent with 5+ years relevant experience.
• Minimum 2 years relevant experience preferred.
• Experience with homelessness, survivors of domestic violence, mental health or addictions challenges, and knowledge of community resources.
• Experience with the Wisconsin Homeless Management Information System, Clarity.
• Valid Wisconsin driver’s license and clean driving record; ability to travel locally.

Responsibilities

• Provide primary crisis problem solving and intervention.
• Builds and maintains a strong connection with all clients using a strength based, trauma informed approach.
• Acts with a sense of urgency and supports clients in finding housing to avoid shelter entry.
• Completes thorough assessments to mutually establish goals for a client’s tenure.
• Serves as an advocate for participants across systems and brokers between resources.
• Manages records and grant documentation for Pillars services.
• Represents the agency in community or interagency activities.
• Other duties as assigned.
