Job summary

Pillars is hiring a full-time Diversion and Prevention Case Manager in Appleton, WI. You will assess clients in temporary housing and provide case management to transition them to affordable, stable housing while working beyond traditional shelter settings.

The role emphasizes crisis intervention, advocacy, record-keeping, and collaboration with community resources, with a strong mission to prevent shelter entry.

Qualifications

• Bachelors in Human Services or equivalent with 5+ years relevant experience.
• Minimum 2 years relevant experience preferred.
• Experience with homelessness, domestic violence, mental health, or addictions support is a plus.

Responsibilities

• Provide crisis intervention and problem solving for clients.
• Assess clients and set mutually agreed goals for tenure in housing.
• Advocate for participants across systems and refer to community resources.
• Maintain paper and electronic records and grant documentation.
• Coordinate housing-related services and document outcomes.
• Represent the agency in community events and interagency activities.
