Hartford HealthCare is seeking a part-time Discharge Lounge RN at Hartford Hospital to facilitate patient throughput by coordinating the transfer of discharge-ready patients to the Transition Lounge. You will work with unit nurses, Case Coordination, and support services to move patients smoothly and safely.

• Current RN license in Connecticut.
• Experience with discharge planning or patient flow.
• Experience with care coordination or case management.

Responsabilidades

• Coordinate transfer of discharge-ready patients from inpatient units to the Transition Lounge.
• Collaborate with unit nurses, Case Coordination, and support services to transition appropriate patients.
• Serve as a central resource for discharge throughput and patient flow initiatives.
• Provide clinical oversight for patients awaiting transportation or post-discharge services.
• Deliver handoff communication and report to EMS and transportation providers.
• Oversee daily operations and workflow within the Transition Lounge.
• Lead and support Transition Lounge staff to maintain a safe and efficient environment.
• Monitor and address barriers to discharge flow, escalating concerns as appropriate.
• Ensure compliance with organizational policies and patient safety standards.
• Promote Hartford Health Care values through professional communication and teamwork.
