Qualifications

• Three (3) years of healthcare experience.
• Strong analytical, data management and computer skills.
• Current working knowledge of discharge planning, Medicare, Medicaid and managed care reimbursement, as well as all rules and regulations governing Home Health.
• Ability to work independently and exercise sound judgment in interactions with physicians, payors, and patients and their families.
• Promotes quality patient care and customer service/satisfaction, while promoting safety, cost efficiency, and a CQI mindset.

Responsibilities

• Collaborates and maintains active communication with physicians, nursing staff and other members of the multi-disciplinary care team, patients and their families, and other Home Health companies.
• Proactively identifies and resolves delays and obstacles to discharges with ordered durable medical equipment.
• Collects and informs patients and families of all co-pays due; processes monies collected.
• Consults with clinical staff to ensure HH orders are valid and complete.
• Coordinates delivery of all durable medical equipment to patient homes and demonstrates safe and proper use.
• Collaborates with the physician and team to facilitate care for designated caseload and monitor progress toward discharge.
• Ensures plan of care elements are communicated to patient/family and healthcare team for continuity of care.
