Job summary

Memorial Hermann Health System seeks a Licensed Master Social Worker to provide clinical social work and complex discharge planning for patients with psychosocial needs. Under supervision, you will deliver crisis intervention, assess mental health, and coordinate multidisciplinary discharge plans with physicians, case managers, and care teams.

Qualifications

• Graduate of an accredited Masters of Social Work program (MSW)
• Current LMSW license in Texas required; ACM certification from ACMA preferred

• Provides crisis intervention and mental health assessment as needed.
