Job summary

Mission Community Hospital is seeking a compassionate Licensed Vocational Nurse (LVN) to join the Case Management Department. The role focuses on discharge planning, coordination of post-acute care, and ensuring timely transitions of care for hospitalized patients.

The ideal candidate has SNF experience and strong EMR skills, with collaboration across physicians, nurses, social workers, and the Case Management team. This is a full-time, onsite position.

Qualifications

• Must hold a current California LVN license in good standing.
• Minimum of one year SNF experience.
• Current BLS certification.
• Knowledge of discharge planning and post-acute care preferred.
• Strong organizational, communication, and documentation skills.
• Ability to collaborate with physicians, nurses, social workers, and interdisciplinary teams.
• Proficiency in EMR.

Responsibilities

• Assist Case Management with discharge planning and post-acute care coordination.
• Coordinate referrals to SNFs, home health, rehab, hospice, DME, and community resources.
• Communicate with insurers and providers to facilitate timely transitions.
• Review clinical documentation to support discharge planning with continuity of care.
• Monitor patient progress and identify discharge barriers.
• Collaborate with physicians, nurses, social workers, and therapists for timely discharges.
• Maintain accurate EMR documentation.
• Assist with prior authorizations and payer coordination as assigned.
• Educate patients and families on post-discharge services.
• Participate in interdisciplinary rounds and Case Management meetings.
• Ensure HIPAA compliance.
• Perform other duties as assigned.
