Job summary

Responsibilities

• Coordinate discharge planning and social services for patients and families.
• Collaborate with the Medical Staff and care team to develop comprehensive discharge plans.
• Monitor information to support assessment, treatment, education and continuity of care.
• Complete insurance pre-certifications for psychiatric transfers and DMEs as needed; document in MEDITECH/MIDAS.
• Document care plans and ensure timely and compliant discharge planning.
