Job summary

Northwestern Medicine is seeking a Social Worker II Flex to collaborate on discharge planning and psychosocial care for high-risk patients and families with complex needs. This role requires expertise in coordinating screenings, assessments, and coordinating with care teams to ensure safe transitions.

Responsibilities include educating patients about resources, linking to community services, providing on-call coverage, and maintaining thorough documentation under department policies.

Qualifications

• Masters Degree from an accredited School of Social Work or Bachelor's Degree in Social Work and three years of experience in medical setting preferred and enrolled in MSW program preferred.
• Must have good knowledge of theory and practices of social work; understand community health and social agencies.
• Licensure in Illinois; Licensed Social Worker or Licensed Clinical Social Worker (LCSW).

Responsibilities

• Discharge planning and psychosocial interventions for patients and families.
• Identify patient's psychosocial and discharge planning needs.
• Know community resources to support interventions and meet patient needs.
• Educate patients/families about community resources and involve them in intervention planning.
• Support patient choice by informing about options/resources and rights to choose providers.
• Document progress per policies, including interactions and resources.
