Job summary

Catholic Health Services is seeking a Social Worker to coordinate discharge planning for residents, address psychosocial needs, and liaise with physicians and nurses to monitor progress.

Responsibilities include conducting psychosocial assessments, educating residents on rights and resources, and guiding families through care planning, with documentation in the clinical software system.

Qualifications

• MSW, BSW or Bachelor’s Degree in a Human Services field.
• 1 year supervised Social Work experience in healthcare.
• CPR Certification.

Responsibilities

• Interview new admissions and complete psychosocial history.
• Coordinate discharge planning and community resources.
• Attend care plan meetings with the interdisciplinary team.
• Provide counseling to residents and/or family.
• Document progress notes in the medical record.
• Ensure PASRR and Medicare program enrollment where applicable.
