Job summary

Trinity Health seeks a dedicated social worker to participate in discharge planning, develop social care plans, and assess residents. You will coordinate with families and community partners to ensure smooth transitions and optimal placement.

The role includes communicating progress to residents and families, providing information on Medicare/Medicaid, and consulting with staff to address resident concerns through social service programs.

Qualifications

• Bachelor's or Master's degree in social work or a human services field.
• One year of supervised social work experience in a healthcare setting.
• Must be licensed as a social worker in Michigan.

Responsibilities

• Participate in planning related to resident, family, and community interests; participate in discharge planning; develop and implement social care plans and resident assessments.
• Promotes optimum transition of resident for appropriate and safe placement for discharge; maintain ongoing communication with resident and responsible persons.
• Provide information to resident/family on Medicare/Medicaid and other financial assistance; consult with staff and community agencies to solve resident concerns through social service programs.
