Responsibilities

• Support comprehensive patient planning and post-discharge coordination to promote safe transitions of care.
• Screen and assess patients and families for social drivers of health and resource needs.
• Serve as an expert resource regarding community-based social services and social determinants of health resources.
• Connect patients and families with community programs and services.
• Refer patients to Behavioral Health when appropriate.
• Provide education and support related to food access, housing resources, and other essential needs.
• Coordinate patient transportation and access to care.
• Assist patients and families in crisis by helping understand medical implications.
• Provide support and counseling regarding impact of conditions on daily life.
• Conduct financial assessments and arrange in-home care and equipment as needed.
• Collaborate with healthcare team to advocate for patients and reduce barriers to care.
• Maintain documentation and follow standards for social work services.
