Job summary

SanStone Health seeks a licensed social worker to assist in planning, developing, and directing the facility's social service programs. You will interview residents and families, assess social needs, and help coordinate discharge planning and community referrals.

You will collaborate with medical staff, administrators, and outside agencies to ensure compliant policies and high-quality resident care, while staying current with federal and state regulations and professional standards.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Assist in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview resident/families to obtain social history.
• Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director as required.
• Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
