Job summary

You will review records, collaborate with IDT, and ensure compliance with standards. You will develop discharge plans, coordinate post-hospital placement, and serve as a liaison with patients, families, and providers while maintaining thorough documentation and timely follow-up.

Qualifications

• Associate Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• 2–4 years of clinical nursing experience in hospital/home health/nursing home settings required.
• 2–4 years of care management experience preferred.

Responsibilities

• Coordinates discharge planning and transitions of care.
• Reviews medical records for appropriateness and medical necessity.
• Collaborates with interdisciplinary teams to ensure care continuity.
• Develops and implements discharge plans and post-hospital placement.
• Maintains accurate documentation and compliance with reporting requirements.
• Interacts with patients, families, and physicians regarding post-hospital care options.
