Job summary

Roosevelt Care Center in Old Bridge, NJ is hiring a Discharge Planning Coordinator (LPN or CSW) to oversee the discharge process for the Sub-Acute Unit and help residents transition safely to the next level of care.

The role requires an active NJ LPN or Social Work license and at least four years of social work or clinical nursing experience, with long-term care or rehab experience preferred. Flexible scheduling and a supportive team are offered.

Qualifications

• Active NJ LPN or Social Work license.
• Minimum 4 years of social work or clinical nursing experience.
• Long-term care or rehab experience preferred.

Responsibilities

• Coordinate and oversee resident discharge planning for the Sub-Acute Unit
• Participate in utilization review and insurance/Medicare reviews
• Issue Medicare cut letters as needed
• Collaborate with interdisciplinary teams on treatment and discharge plans
• Provide social service support to residents and families
• Coordinate community resources and post-discharge services
• Maintain accurate records and documentation
