Job summary

Care Connection Rehabilitation Center in Rahway, NJ is seeking an experienced Social Worker to join our skilled nursing and rehabilitation team. The Social Worker will oversee social work, care planning, and discharge planning throughout the resident's stay.

The ideal candidate has at least 2 years of SNF experience, excellent communication, and hands-on discharge planning with PCC EMR.

Qualifications

• Minimum 2 years of social work experience in a nursing home/SNF or rehabilitation setting.
• Experience with discharge planning and transitions of care.
• Experience using PCC EMR (PointClickCare).
• Excellent verbal and written communication skills.

Responsibilities

• Provide comprehensive social work services and psychosocial support to residents and families.
• Oversee social work process for all admissions and discharges.
• Coordinate discharge planning from admission through transition to the community or next level of care.
• Identify barriers to discharge and develop solutions and resources.
• Coordinate community resources, home care, and transportation as needed.
• Complete required social work assessments, care plans, progress notes, and follow-up.
• Participate in interdisciplinary care planning meetings and collaborate with nursing, rehab, and medical staff.
• Advocate for resident needs, rights, and quality of life.
• Maintain communication with residents, families, and outside agencies.
• Monitor compliance with regulatory requirements related to social work services.
• Maintain accurate and confidential resident records in PCC EMR.
• Provide continuity of social work services from admission through discharge.
• Support residents and families in understanding care options and discharge plans.
