Job summary

Roosevelt Care Center at Old Bridge is seeking an experienced Discharge Planning Coordinator (LPN or CSW) to oversee the discharge process for the Sub-Acute Unit and help residents transition safely to the next level of care. This on-site role requires active NJ licensure in nursing or social work and a strong background in discharge planning.

You will collaborate with interdisciplinary teams, participate in utilization review, and provide social services support to residents and families,

Qualifications

• Active NJ Practical Nurse (LPN) or Social Work license.
• Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning.
• Long-term care or rehab experience preferred.

Responsibilities

• Coordinate and oversee resident discharge planning for the Sub-Acute Unit.
• Participate in utilization review and insurance/Medicare reviews.
• Issue Medicare cut letters as needed.
• Collaborate with interdisciplinary teams on treatment and discharge plans.
• Provide social service support to residents and families.
• Coordinate community resources and post-discharge services.
• Maintain accurate records and documentation.
