UPMC Mercy in Pittsburgh is hiring a Casual Discharge Plan Manager to support the Clinical Care Coordination and Discharge Planning Department. The role involves coordinating post-discharge needs, partnering with physicians and care teams, and ensuring timely, safe transitions.

Candidates may be an RN or social worker with at least one year in discharge planning or care coordination. Salary will be determined at offer and will reflect experience.

• One year of discharge planning/care coordination experience required.

Mansioni

• Assess clinical, psychosocial, financial, cultural, and spiritual needs to guide discharge planning.
• Evaluate barriers to discharge and coordinate post-hospital services.
• Facilitate safe and efficient discharges with care teams and post-acute resources.
• Communicate clearly with patients and caregivers and coordinate with the interdisciplinary team.
• Coordinate with hospitals, post-hospital facilities, and physicians for smooth transitions.
• Ensure compliance with regulatory standards and advocate for patient access and rights.
• Provide staff orientation and mentoring as appropriate.
