Job summary

Solaris HealthCare Charlotte Harbor in Port Charlotte, Florida, is seeking a Social Service Associate/Discharge Planner to coordinate the facility’s social services program and meet residents’ emotional and social needs in accordance with federal, state, and local standards.

Preferred qualifications include a bachelor's degree in social work or a related field and at least one year of supervised social work experience in a healthcare setting, with discharge planning experience being a plus.

Qualifications

• Preferably a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology.
• Will preferably have one (1) year supervised social work experience in a health care setting working directly with individuals; discharge planning preferred.

Responsibilities

• Assist in planning, organizing, implementing, evaluating, and directing the social service programs of Solaris HealthCare Charlotte Harbor.
• Participate in discharge planning, resident assessments, and development and implementation of social service care plans.
• Involve the resident/family in planning social service programs when possible.
