Job summary

Admiral Nursing & Rehab, a Vierra Communities property, in Silver Spring, MD, seeks a Social Services & Discharge Planning Assistant to oversee the Social Services Department and coordinate resident discharge planning.

Requirements include a bachelor’s degree in social work or related field, at least one year of supervised social work experience in healthcare, and an active license. Competitive pay and comprehensive benefits are provided.

Qualifications

• A minimum of a bachelor's degree in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology
• One year of supervise social work experience in a health care setting working directly with individuals
• Licensed

Responsibilities

• Oversee development, implementation, supervision, and evaluation of the Social Services Department.
• Identify medically related social services and ensure that these services are provided in accordance with State and Federal regulations.
• Collaborate with the interdisciplinary team on discharge planning and community-based follow-up care
