Job summary

Solaris HealthCare Forest Lake in Apopka, Florida is seeking a full-time Social Services Assistant to support admissions and coordinate center services for patients’ psychosocial needs. The role emphasizes discharge planning, patient advocacy, and collaboration with families and other departments.

Qualified candidates will have at least a high school diploma and a degree in social work or psychology; crisis experience and English proficiency are preferred.

Qualifications

• Bachelor's degree in social work or related field preferred.
• Experience in dealing with people in crisis is preferable.
• Knowledge of casework process and community resources is helpful.
• Proficiency in standard English is required.

Responsibilities

• Admit patients to center following state, federal and company policies.
• Establish and maintain waiting lists and inform relevant parties.
• Assist in room assignments and coordinate cross-department communication.
• Maintain complete and legible patient records in a timely fashion.
• Prepare for and participate in multidisciplinary Patient Care Planning Conferences.
• Act as Ombudsman and inform patients of rights and privileges.
• Record and report patient complaints and suggested remedies to the administrator.
• Conduct basic counseling and coordinate transportation and family involvement as needed.
• Coordinate discharge planning to ensure medically appropriate discharge times.
• Provide information to facilities with Transfer Agreements on discharge.
• Participate in Utilization Review meetings and community resource referrals.
• Develop educational programs regarding nursing home services.
• Engage in continuing education and quality assurance activities.
