San Francisco Post Acute is seeking a compassionate and organized Social Services / Discharge Planner to join our team full-time in San Francisco, CA. The role focuses on supporting residents and families through care journeys, coordinating discharge planning, and connecting them with community resources to ensure safe transitions.

Qualifications include a Bachelor’s in Social Work or Human Services (MSW preferred) and at least two years of supervised social work experience in healthcare.

Responsabilités

• Assess residents' social and family needs to create individualized social services plans.
• Provide emotional support and counseling to residents and families.
• Coordinate discharge planning with residents, families, healthcare providers and the team.
• Arrange referrals and community services for safe transitions.
• Help residents understand care options and healthcare decisions.
• Coordinate outpatient appointments and on-site services.
• Maintain documentation of residents' needs and discharge plans.
• Build relationships with facility departments and community partners.
• Provide staff education and in-service training.
