The role requires two years of related experience, CPR/CPI/First Aid, and reliable transportation for client transport. Telecare emphasizes equity, inclusion, and high-quality behavioral health services for diverse populations.

Formación

• Two years of professional experience in Social Work, Nursing, or a related field.
• Must be at least 18 years of age.
• CPR, CPI, and First Aid certifications required on date of employment or within 60 days.

Responsabilidades

• Provide discharge planning services coordinating with care teams and agencies.
• Collaborate with Conservator's Office, eligibility workers, Public Defender, Public Guardian, and Court Services.
• Coordinate services across county mental health system and ensure smooth transitions for clients.
