Job summary

Lakeside Behavioral Health System is seeking a Discharge Planner to provide discharge planning, case management, and crisis intervention within an inpatient psychiatric setting. This role involves liaison work with referrals and hospital departments to ensure smooth patient transitions.

Qualifications

• Bachelor's degree in Social Work, Clinical/Counseling, or related social science field.
• Experience with psychiatric mental health population; inpatient setting preferred.
• RN licensure/registration if applicable (Tennessee Board of Nursing).
• Knowledge of case management, discharge planning, and family therapy theory.

Responsibilities

• Provide discharge planning and case management services.
• Perform crisis intervention and liaison services within the hospital.
• Coordinate with referrals and other departments to align patient care.
• Support staff and families through transition and care planning.
