Job summary

Lakeside Behavioral Health System in Memphis, TN, is seeking a Discharge Planner to join a team focused on compassionate patient care. Qualifications

• Bachelor’s degree in Social Work, Clinical/Counseling, or Social Science related area required, licensure preferred.
• Graduate of an accredited nursing program.
• Experience with psychiatric mental health population, preferably inpatient.

Responsibilities

• Provide discharge planning and case management services, crisis intervention and liaison with referral sources and hospital departments.
• Support reassignment to varied populations including adults and dually diagnosed in inpatient and partial hospital settings.
