Job summary

Perimeter Behavioral of Springfield is seeking a compassionate Discharge Planner to help children, teenagers, and families move smoothly from treatment to aftercare. You will join a patient-centered team that collaborates across therapies, case management, and care coordination to ensure seamless transitions.

The role requires a Bachelor’s degree in social work or a related field, 1–2 years in mental health settings, CPR and CPI training within 30 days, and a Monday–Friday schedule from 8:00

Qualifications

• Bachelor’s degree in social work, psychology, or a related field.
• One to two years of experience in an inpatient/outpatient mental health setting, or comparable experience; strong knowledge of clinical treatment principles and social casework preferred.
• Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment.
• Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.

Responsibilities

• Participate as an active member of the treatment team, including staff meetings, treatment planning meetings, and assigned committees.
• Coordinate with therapists, utilization review staff, doctors, and insurance companies to support documentation, discharge planning, and continuity of care.
• Help identify patient needs and work with the treatment team to ensure those needs are addressed.
• Partner with families and treatment teams to encourage involvement in each patient’s care.
• Communicate with referral sources, mental health professionals, and community agencies to support aftercare planning and successful transitions.
