Job summary

Perimeter Behavioral of Springfield is seeking a compassionate Discharge Planner to guide children, teens, and families through treatment and into successful aftercare, collaborating with the treatment team.

You will coordinate with therapists, physicians, and insurers to plan discharges, identify patient needs, involve families, and ensure smooth transitions to aftercare options. This full-time role follows a Monday–Friday, 8:00 a.m.–5:00 p.m. schedule in our Springfield facility.

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• 1–2 years of inpatient/outpatient mental health experience preferred.
• Successful CPR and Crisis Prevention Intervention Training within 30 days of employment.

Responsibilities

• Participate as an active member of the treatment team, including staff meetings and treatment planning meetings.
• Coordinate with therapists, physicians, and insurance companies to support documentation, discharge planning, and continuity of care.
• Help identify patient needs and work with the treatment team to ensure those needs are addressed.
• Partner with families and treatment teams to encourage involvement in each patient’s care.
• Communicate with referral sources, mental health professionals, and community agencies to support aftercare planning and successful transitions.

What We’re Looking For

• Education: Bachelor’s degree in social work, psychology, or a related field.
• Experience: One to Two years of experience in an inpatient/outpatient mental health setting, or comparable experience; strong knowledge of clinical treatment principles and social casework preferred.
• Training: Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment.
• Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.
