Job summary

• Knowledge of community resources and ability to network within the community.

This position functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards.
