Job summary

AHMC- Anaheim Regional Medical Center is seeking a Discharge Planner to support patients and families with comprehensive discharge planning services. The role involves collaboration with the interdisciplinary team, community outreach, and coordination of resources to ensure safe and effective transitions from hospital to home or another care setting.

The incumbent will apply infection control policies, participate in department education, and contribute to performance improvement initiatives

Qualifications

• Bachelor’s degree from an accredited school of Social Work or related field (preferred).
• RN or LVN (preferred).
• Two years acute or related medical care experience.

Responsibilities

• Provide discharge planning assessments and interventions for patients in a timely and appropriate manner.
• Develop and implement discharge planning plan of care.
• Advocate for patient rights and connect with community resources as needed.
• Maintain documentation and statistical records.

Job description

The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.

The Discharge Planner is an integral part of the interdisciplinary team, and also provides community outreach, networking, and liaison with community programs as needed.

Consistently applies infection control policies/practices.

Meets population specific competencies.

Attends department specific education/training, inservices, staff meetings.

Department specific performance improvement project.

Demonstrates ability to provide timely and appropriate discharge planning assessments and interventions for patients .

Demonstrates ability to develop and implement appropriate discharge planning plan of care.

Demonstrates ability to advocate for patient’s rights.

Knowledgeable regarding community resources and demonstrates ability to mobilize them on patient’s behalf.

Demonstrates ability to maintain appropriate documentation and statistical records.
