Job summary

North Canyon Network of Care is seeking a Discharge Planner to identify patient needs and priorities for care through assessment, planning, intervention and evaluation. You will coordinate care between professional disciplines and provide psychosocial support to patients and families facing chronic, acute, or terminal illness.

The role involves advising family caregivers, counseling patients, and helping plan for discharge needs to ensure smooth transitions of care.

Qualifications

Requirements

• Current BLS certification.
• ACLS and PALS recommended but not required.
• Ability and willingness to demonstrate and maintain competency as required for job title and the unit/area(s) of assignment.
• Excellent communication skills to include written and verbal comprehension and expression.
• Ability and willingness to work with patients’ growth and development needs particularly related to the age of patients in the unit/area(s)of assignment.
• Ability to manage a chaotic work environment related to changing patient needs, including working with patients with acute, chronic, and complex disease processes, and those that are dying.
