AHMC Anaheim Regional Medical Center is seeking a Discharge Planner to coordinate patient discharge planning across hospital areas, aligning with customer service expectations and ongoing care needs. You will work with patients, families, and the interdisciplinary team to assess needs and arrange appropriate services.

The role emphasizes community resource navigation, documentation, and advocacy for patient rights, with preferred qualifications in social work or nursing, and at least two years

Qualifications

  • Bachelor’s degree from an accredited school of Social Work or related field (preferred), RN or LVN (preferred)
  • Two years acute or related medical care experience.

  • Knowledgeable regarding community resources and demonstrates ability to mobilize them on patient’s behalf.
  • Demonstrates ability to maintain appropriate documentation and statistical records.

Job description

Overview

The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.

The Discharge Planner is an integral part of the interdisciplinary team, and also provides community outreach, networking, and liaison with community programs as needed.

Responsibilities

Consistently applies infection control policies/practices.
